Regardless of your browser choice, it must support 128-bit encryption. The level of encryption has been increased in order to enhance security. * Definition of a School

* Requests for new school codes, school closures, name changes and grade changes for the 2012-13 school year are due to CDE by Friday, June 29. This information is crucial to ensure accurate Title IA funding, NCLB ESEA waiver implementation, on-time and accurate release of the School Performance Frameworks and earlier dissemination of the pre-populated Unified Improvement Plans. Additionally, having a final 2012-13 school list earlier will allow for better reporting in SchoolView and access to grade specific assessment materials.

* In order for the Colorado Department of Education to assign a 4 digit code to a new school, a school district must submit a Request for New School Code. Once the form has been filled out, FAX the request to the attention of Dennis St. Hilaire at (303) 866-6888. A new school code will be issued once the information is received

* If a school is closing, notification from the superintendent needs to be sent to the data and research unit at CDE, so we may properly close the school for all data submissions.

* If there are changes to a school name or changes to school grade range, a letter (on district letterhead) or a completed form below from the district superintendent is required indicating the changes. Once the letter is signed by the superintendent, FAX the letter to the attention of Dennis St. Hilaire at (303) 866-6888.

* Adequate Documentation of Transfer List - List that contains exit codes, exit type codes, and the documentation needed for transfer students to avoid incorrectly coding students as dropouts.

Note:  Each of these forms can be modified to suit the needs of individual districts and schools.  At a minimum, the text in green on the records request and confirmation of enrollment forms should be overwritten with your district's information before use.  The forms can then be printed and the information can be handwritten, or they can be stored electronically and the information can be typed directly onto the form.  If you wish to use the forms electronically, you should first open the form (in Microsoft Word) then select "View" ->  "Toolbars"  -> "Forms", then select the icon that looks like a padlock to protect the form.  This will allow you to utilize the embedded pull-down menus and to easily tab between the data entry spaces on each form.
